Your Child's Record Book

The following should be included in your Child's Record Book.  This will be extremely helpful for children transitioning from one school/program to another.  This will also help in the writing of goals, either for a 504 plan, or an IEP (Individualized Educational Plan).

• Profile:  Notes on your child's strengths, weaknesses, communicating style, likes and dislikes.

• History:  Notes stating your child's family tree, health history, developmental history and any significant events in your child's life.

• Current concerns:  Document any concerns you have before each meeting with a professional.  This can be the basis for your discussion.  Date these records.

• Past concerns:  Keep past records, again dated, and include any action taken.

• Medical:  File in this section copies of any letter you have written to or received from doctors, etc.

• Diagnostic of Treatment Reports:  Record any relevant medications your child has been prescribed, and indicate changes in medication here.  Also file in this section any articles or information you have shared with health professionals.

• Psychological/Social:  File in this section any copies of letters, reports, assessments, etc. dealing with psychological/emotional/social/behavioral concerns.  If the document relates to school, put a note in the "Educational" file to say that this information is in the "Psychological" file.

• Educational:  File here copies of anything about your child in school in the past couple of years.  Include report cards, letters, notes, assessments, teacher rating scales, descriptions of your child as a learner, reports or other documents about case conferences, staffings, and IEPs.
